A California-based program is seeking vendors to deliver senior injury prevention and health promotion services. The selected provider will be responsible for conducting outreach, orientation, and screening activities to identify older adults eligible for these services, with a particular focus on individuals residing in medically underserved areas or facing significant economic challenges.

Key responsibilities include providing employment opportunities that allow participants to use their English and additional language skills in serving clients, conducting follow-ups required by evidence-based models (such as Otago continuation calls, home modification checks, medication follow-ups, and program-mandated coaching and reassessments), and developing an Emergency Operations Plan that addresses facility preparedness, staff and volunteer training, and fire safety procedures. The vendor will also be required to maintain accurate statistical and financial records to support program and financial reporting on a monthly basis.
