The Rural Health Transformation: Building Dementia Capable Communities program is a new initiative for rural communities in Eastern Washington. The program aims to provide services to support individuals with dementia and their caregivers. The initial contract is intended for both program development and initial service implementation, with the program intended to be a five-year funded program. The initial contract may include an option for extension, subject to satisfactory performance, an approved scope of work and budget, and written approval by ALTCEW.

Must have at least one year of experience providing the same or similar services in Washington State
Must obtain prior written approval from ALTCEW to subcontract services
Must provide proof of active general liability insurance before the contract is executed
