(1) Vendor needs to provide professional community outreach service. – Professional community outreach experts to support its efforts in public education, resource linkage, and patient navigation. – Provide the outreach materials and assist with high level community data for developing goals and priorities. – The importance of breast and cervical cancer screening, connect women to available services and barrier relief through B+C, and provide navigation support to priority populations to ensure successful completion of recommended screenings. – Inform individuals of the B+C screening assistance program. – Educate women about breast and cervical cancer screening – Link individuals to B+C program providers for cancer screening services – Navigate enrolled participants to complete breast and cervical cancer screening (2) The contract period will be for one year. (3) All the questions must be submitted no later than January 5,2026.
