Vendor needs to provide peer support outreach specialist services. Provide peer–based outreach, engagement, and support services. Provides outreach, engagement, and supportive services to individuals experiencing behavioral health conditions, substance use disorders, homelessness, housing instability, poverty, or other social service needs to individuals and families in crisis. Maintain strict professional boundaries and adhere to peer support ethics. Engage individuals exhibiting behavioral health or substance use symptoms and connect them to appropriate services. Make referrals and coordinate service delivery across behavioral health, medical, housing, benefits, and social service crisis support systems. Assist individuals in accessing shelter, food, clothing, medical care, substance use treatment, mental health services, and other essential supports. Assist in establishing, tracking, and reporting program metrics, including outreach contacts, referrals, placements, and outcomes. All questions must be submitted no later than February 11, 2026.
