DESCRIPTION

The selected vendor will provide peer outreach services, employing two full-time peer outreach specialists. These specialists will conduct field-based outreach and engagement with individuals experiencing homelessness, including those with involvement in the justice system. Their responsibilities involve supporting participant engagement and retention in various services, assisting with housing navigation, and facilitating behavioral health and substance use treatment.

Additional services include access to primary and specialty medical care, enrollment in benefits such as Medi-Cal, SSI/SSDI, and CalFresh, as well as support for legal, reentry, and re-stabilization needs. Peer outreach staff are expected to employ trauma-informed, harm reduction, and culturally responsive practices throughout their work, collaborating with agency staff to support care planning, follow-up, and coordination of warm handoffs.

The vendor must provide flexible, client-centered support resources designed to reduce immediate barriers to engagement and stabilization for highly vulnerable populations. Appropriate levels of both programmatic/clinical and administrative supervision for peer outreach specialists are required. A pre-bid meeting is scheduled for June 15, 2026, and all questions must be submitted by June 16, 2026.
