(1) Vendor needs to provide parent support outreach program and family community partnership services.– Provide parenting/family support and case management.– Services are focused on family strengths and needs, and helping families facing challenges such as homelessness, financial hardship, or parenting concerns.– Assessment of needs: Program workers conduct comprehensive assessments to identify family strengths and needs.– Support for basic needs: Assistance with housing, transportation, childcare, and other essential needs that promote child safety and wellbeing.– Families are identified through screened–out child maltreatment reports, community referrals, or self–referrals.– The family must have at least one child aged 17 or younger, or currently pregnant. (2) All the questions must be submitted no later than December 29, 2025.
