A qualified vendor is sought to provide outreach, case management, and disability advocacy program services to individuals in California who are homeless or at risk of homelessness. The selected provider will use current Point in Time (PIT) count data to identify highly populated areas in need of targeted outreach efforts for this population.
Services will be directed toward individuals who are likely eligible for disability benefits and require support to address their housing instability. Responsibilities include completing Homeless Management Information System (HMIS) intake forms, assessing and addressing case management needs, and serving up to 150 individuals with outreach requirements. The program prioritizes collaboration, requiring the establishment of connections with existing outreach teams and partner agencies to enhance the reach and effectiveness of services.
The vendor will also offer transportation assistance for eligible participants, supporting their access to medical appointments, the collection of vital documents, housing searches, and other essential appointments. This initiative aims to comprehensively support homeless or at-risk individuals in accessing disability-related benefits and necessary case management services.
