The selected provider will support youth transitioning from custody or remaining out of custody through individualized planning, coordinated services, and direct case management designed to improve long-term outcomes.

Scope of work includes re-entry planning and service coordination in collaboration with the Deputy Probation Officer, including development of individualized case plans responsive to each youth's needs.
