DESCRIPTION
The selected vendor will provide housing-focused street outreach services for people experiencing unsheltered homelessness. Services will include direct field engagement, safety and stabilization support, and connections to housing, homeless services, behavioral health, public health, and social service resources. Outreach must be person-centered, culturally responsive, nonjudgmental, and autonomy-centered, with an emphasis on progressing individuals toward stable and permanent housing.
Housing-focused outreach teams will conduct consistent, targeted, and ongoing engagement at encampments, street locations, shelters, and drop-in points as appropriate. The teams will also engage individuals living with substance use disorders and facilitate rapid access to low-barrier substance use treatment, including medications for addiction treatment.
The services will focus on reaching high-need priority populations, increasing access to substance use disorder services and housing interventions, applying evidence-based and community-defined practices, and supporting behavioral health workforce development. Vendors will connect people experiencing homelessness to the broader system of care, conduct approved assessments, and prioritize revisions or decliner forms when appropriate.
