The state of Minnesota is seeking a qualified vendor to develop and implement an evaluation and monitoring plan for a health equity strategy. The selected provider will assess the effectiveness of the Health Equity Strategist model, focusing on foundational capabilities, changes in practices, and the development and implementation of division-specific strategies aimed at improving health outcomes.

Responsibilities include designing a comprehensive evaluation and monitoring plan, preparing a final report outlining the process, findings, and recommendations, and creating a roadmap with tools utilizing a maturity model approach for the continued development of the Health Equity Strategist model within agency divisions. The scope also involves reviewing and analyzing all relevant model documents, current evaluation plans, trainings, and other resources, as well as identifying key health equity transformation domains aligned with the strategy model.

Applicants should have substantial experience in research, assessment, and evaluation of public health initiatives, with demonstrated expertise in implementing evaluations of health equity frameworks.
