(1) Vendor needs to provide health equity strategy evaluation and monitoring plan service. • Develop and implement an evaluation and monitoring plan to assess the value of the health equity strategy model in building foundational capabilities, changing practice, and developing, supporting, and implementing division–specific strategies to improve health outcomes • Develop a Health Equity Strategist model evaluation and monitoring plan and a final report outlining the evaluation and monitoring plan process, findings, and recommendations. • Create a roadmap with tools based on a maturity model approach for development of the Health Equity Strategist model within agency divisions • Review and analyze all relevant model documents and materials including concept, current evaluation plan, trainings and resources developed, and other items as identified by staff. • Identify health equity transformation domains in alignment with the model. • Public health practitioner with experience in research, assessment, and evaluation of public health initiatives.
