DESCRIPTION

The New York State Council on Developmental Disabilities (CDD) is seeking a grantee to develop, pilot, implement, and evaluate a comprehensive grief support toolkit for people with developmental disabilities (DD), their families, and caregivers. The project includes creating accessible, culturally competent resources such as visual communication tools, guided conversation prompts, and training programs. The grantee will also develop a dissemination plan and evaluate the project's effectiveness using qualitative and quantitative data. The contract term is two years, with an anticipated start date of April 1st, 2027, and an end date of March 31st, 2029.

KEY REQUIREMENTS

Must be a prequalified vendor in the Statewide Financial System (SFS)
Must be a nonprofit organization (lead applicant)
Must include a minimum of 34% match funding from non-federal sources
Must submit Program Specific Questions, a Work Plan, and a 1-Year Expenditure Budget
