Vendor needs to provide food is medicine infrastructure initiative strategy service. – Professional services for outlining strategies to expand access, improve health outcomes, advance equity, and build workforce and partnerships, and establish replicable, sustainable food is medicine (FIM) infrastructure. – Support the implementation of the FIM initiative in rural areas. – Develop and implement sustainable billing mechanisms. – Develop and deploy Electronic Medical Record (EMR)–integrated billing tools for participating providers. – Establish a rigorous evaluation framework to track clinical outcomes, program reach, cost savings, and value–based payment success. – Prepare reports and publish findings in peer–reviewed journals. – Support agency in securing value–based payment arrangements. – Provide individualized nutrition counseling, education, and care coordination for patients with diet–sensitive chronic conditions. – Implement a six–month pilot program for 375 participants in rural with diabetes or hypertension. – Provide data aligned with outcome measurement goals and standardized metrics for statewide reporting. – Coordination and stakeholder engagement. All questions must be submitted no later than February 27, 2026. The contract period will be for one year. Non–mandatory pre–bid meeting will be held on March 13, 2026.
