Delaware is seeking professional strategy services to advance the state’s Food Is Medicine (FIM) Infrastructure Initiative, particularly focused on rural health outcomes. The selected vendor will provide expert guidance in outlining and executing strategies to expand access to FIM programs, improve health outcomes, enhance equity, and strengthen both the workforce and community partnerships. Essential tasks include supporting the implementation of sustainable FIM infrastructure in rural regions and developing replicable processes that can be maintained long-term.
The scope of work also involves designing and deploying sustainable billing mechanisms, including EMR-integrated tools for providers to facilitate efficient billing. The vendor will establish a rigorous evaluation framework to monitor clinical outcomes, assess program reach and cost savings, and track value-based payment arrangements. Responsibilities further include delivering individualized nutrition counseling, patient education, and care coordination, especially for individuals with diet-sensitive chronic conditions such as diabetes or hypertension.
A six-month pilot program targeting 375 rural participants will be implemented as part of this initiative. The vendor will be tasked with collecting and reporting data aligned with outcome measurement goals and standardized statewide metrics. Additionally, the role includes preparing comprehensive reports, publishing results in peer-reviewed journals, securing value-based payment opportunities, and engaging stakeholders through effective coordination. The contract period is one year.
