(1) Vendor needs to provide evidence–informed strategy technical assistance support service. Agency supports the implementation of evidence–informed strategies that strengthen chronic disease prevention and management capacity, improve use of health information technology, and support performance improvement within participating health systems. This project partners technical assistance support organizations with clinics to implement activities related to practice facilitation and practice transformation, as well as data quality improvement and health information technology utilization, to enhance care delivery, coordination, and performance monitoring. The Contractor shall complete quarterly progress reports with an agency provided template. The Contractor shall participate in check–in calls with the Designated agency Contact to discuss project successes and challenges. Agency will provide the Contractor with the Data Collection Tool Template. Agency will provide the Contractor with the Quarterly Progress Report Template. The Contractor shall use all Agency–required forms. (2) All the questions must be submitted no later than February 23, 2026. (3) The contract period will be for one year.
