California is seeking a vendor to provide a comprehensive enhanced care management solution that advances the state's strategic goals of improving service access and care coordination for residents. The desired platform should integrate existing platforms and systems to create a unified system of record, enabling seamless collaboration across various county departments such as health services, human services, probation, and sheriff, as well as with external providers.
The solution must allow for secure, real-time data exchange to eliminate silos and support robust tracking of client participation across multiple programs and initiatives. Key requirements include capabilities for billing and claims processing—such as generating claims reports and invoices and integrating with billing clearinghouses—along with automated eligibility checks within client records.
