(1) Vendor needs to provide enhanced care management solution • A comprehensive system of record to advance its strategic goals of improving access to services and care to residents through the integration of existing platforms and systems. • Solution that facilitates seamless collaboration across departments and external providers by eliminating data silos and enabling secure, real–time data exchange. • The platform must support robust program tracking, allowing the county to monitor client participation across multiple initiatives such and data systems across several county departments, including health services, human services, probation, and sheriff. • Billing & Claims Processing: Vendor of choice should have capability to generate claim reports and invoices by pulling service data integrated billing clearinghouse or capability to integrate into the instance of a billing clearinghouse of choice. • Automated Eligibility: Vendor of choice must have capability to support automated payor eligibility within the patient’s chart or record • Ability to build in custom client consent forms for service delivery and data sharing between care team providers. • End–User Permissions and Accessibility: Must have capability to grant varying levels of user access permissions to client information, such as Protected Health Information (PHI) ranging from a limited view–only to full access (2) All questions must be submitted no later than October 31, 2025
