SUMMARY

The City of Phoenix seeks a provider for employee assistance, elder care, trauma counseling, and substance abuse services for staff, including onsite, virtual, and telephonic options.

KEY REQUIREMENTS

Must have a minimum of 5 years of experience in administering an Employee Assistance Program (EAP), including Elder Care, Traumatic Event Counseling, and Substance Abuse Professional Services.
Must have a minimum of 5 consecutive years of experience providing services for private and public entities comparable in size to the City of Phoenix.
Must have a minimum of 300 EAP network behavioral health professionals within Maricopa County.
Must be lawfully authorized to conduct business in Arizona.
Must be HIPAA certified.
Must provide a performance bond in the amount of 10% of the total contract amount.
Must maintain specific insurance coverage including Commercial General Liability, Automobile Liability, Workers' Compensation, Professional Liability, and Technology Errors and Omissions Liability.
