(1) Vendor needs to provide electronic medical record solution.– Provide a remote hosted enterprise–wide electronic medical record (EMR) solution for use across all agency facilities.– Enterprise master person index we expect an enterprise master patient index solution to be part of the EMR solution.– Integration and interfaces the software components and functionalities enable the seamless exchange of data between an EMR system and other healthcare systems or applications.– Acute Care solution should serve as the flagship solution across the hospital environment.– Ambulatory care the systems should provide longitudinal patient care, including chronic care management.– Care transition planning management the application should automate the search for appropriate and available care transitions.(2) The contract period will be for ten years.
