Comprehensive Rehabilitative Management Needs Facility Services

The government authority in Indianapolis, Indiana, is seeking a provider of comprehensive rehabilitative management needs facility services. The provider must have the operational capacity to scale staffing, resources, and facility supports for up to 20 individuals and serve a diverse population with varying abilities, behavioral needs, and support requirements. Services must follow person-centered planning principles and support individuals in the least restrictive environment appropriate to their needs.

The provider must maintain contingency plans for staffing shortages, emergencies, technology outages, and other operational disruptions to ensure continuity of care. Staff must remain awake and attentive to facility needs on a 24-hour basis, receive structured onboarding and ongoing training in behavioral supports, medical needs, crisis intervention, documentation, trauma-informed practices, and applicable service expectations, and be supervised and routinely assessed for effective performance.

The provider must establish clear and accessible communication processes for individuals, guardians when applicable, family members, employees, service coordinators, and case managers. Written policies and procedures must prohibit and address mistreatment, neglect, abuse, and exploitation of individuals, with performance issues and incidents identified and managed promptly.
