(1) Vendor needs to provide community–based outreach services.– The purpose of this program is to create connections and make referrals to supports and services within the community.– Community outreach services are to assist in building trusting relationships and to work with homeless individuals and families, meeting them where they’re at and assisting them to access appropriate services.– The goal is to have a community partner provide these services alongside municipal staff in a coordinated fashion.– Provide culturally safe and appropriate outreach and housing supports for Indigenous individuals and families who are homeless or at risk of homelessness, including direct connections to Indigenous-specific services, supports, and housing options.– Reduce community impacts of homelessness by responding to public concerns, coordinating with municipal employees and partners, and providing education and information to residents and businesses.– Maintain strategic mapping and monitoring of encampments and homelessness hotspots, with timely and accurate data entry, case notes, and real-time encampment information recorded in Homeless Individuals and Families Information System (HIFIS).– Promote housing stability and retention by providing proactive tenancy-based supports following housing placement.– Conduct proactive foot and vehicle-based outreach across urban and rural areas, including wellness and safety checks.– Provide basic needs support, safety planning, and harm-reduction supplies where authorized.– Maintain strong coverage in rural and remote locations, including seasonal and dispersed sites, and coordinate outreach through libraries, clinics, and other community hubs.– Ensure transportation is available to support outreach activities, service connections, and landlord meetings.– Ensure consistent communication of outreach hours and service.
