DESCRIPTION

The Commonwealth of Kentucky, through the Finance and Administration Cabinet on behalf of the Cabinet for Health and Family Services, is soliciting bids for a Master Agreement for mowing and landscaping services at Central State Hospital, located at 10510 LaGrange Road, Louisville, Kentucky. The scope includes regular mowing, weed eating, bed maintenance, pruning, and debris removal across approximately 47 acres, as well as winter snow and ice removal services. The initial contract period is one year, with four additional one-year renewal options.

KEY REQUIREMENTS

Must have at least five years of experience in the field
Must be registered in the Commonwealth eProcurement System via the Vendor Self Service System
Must provide a list of all employees' names and telephone numbers providing services at the location
Must furnish Material Safety Data Sheet for any chemical used
Must provide proof of Commercial General Liability Insurance with minimum limits of $1,000,000 per occurrence and $2,000,000 aggregate
Must provide proof of Automobile Liability Insurance with a minimum coverage limit of $1,000,000 per occurrence if using vehicles on-site
Must submit Revenue Form 10A100 Kentucky Tax Registration Application
Must submit Certificate of Authority if a foreign entity
