The River Valley Regional Commission Area Agency on Aging is soliciting proposals for the provision of Wellness/Health Promotion and Disease Prevention services. The program aims to assist older adults (aged 60+) and their caregivers in maintaining health, managing chronic conditions, and promoting healthy lifestyles through evidence-based programs such as the Chronic Disease Self-Management Program, Matter of Balance, Tai Chi, and the Diabetes Self-Management Program. Services must be provided across the sixteen-county River Valley region, primarily utilizing senior centers as venues. The contract term is one year, beginning July 1, 2027, and ending June 30, 2028, with options to renew for two additional years through June 30, 2030.

KEY REQUIREMENTS

Must have a minimum of two years of experience providing the proposed service or four years of experience providing services for older adults, caregivers, or another at-risk population
Must be licensed in the State of Georgia to operate a business
Must serve the entire sixteen-county River Valley area
Must provide evidence-based wellness programs
Must maintain a fidelity bond
Must have a financial management system and internal controls
Must comply with HIPAA and sign a Business Associate Agreement
