SUMMARY

KEY REQUIREMENTS

Must submit a completed and signed Representations and Certifications form (PC601).

Must maintain required insurance coverage including Commercial General Liability, Automobile Liability, Workers' Compensation, and Professional Liability.
Must comply with California Revenue and Taxation Code section 18662 regarding non-resident withholding if applicable.
Must be able to perform work within the United States.

DESCRIPTION
