Provider-Facing Population Health, Care Coordination, and Interoperability Solutions

This request for information seeks input on provider-facing population health, care coordination, and interoperability solutions intended for use by rural healthcare providers in North Dakota. The purpose is to collect information about available solution options and the functionality of population health data systems that can support healthcare organizations in managing and exchanging a wide range of health-related information.

Respondents are asked to describe capabilities for receiving and integrating Medicaid claims, encounter, clinical, pharmacy, laboratory, immunization, social care, and administrative data. Solutions should be able to normalize, reconcile, and exchange data from multiple sources, and explain how data is stored, transmitted, processed, and accessed. The requested capabilities also include connectivity with rural hospitals, clinics, specialty providers, behavioral health practices, pharmacies, skilled nursing facilities, and community-based organizations, including environments that use different electronic medical record systems.

The requested information should also address support for bidirectional exchange of clinical, care management, referral, and administrative information needed for provider-facing care coordination. Additional areas of interest include enabling exchange among providers, payers, and community-based organizations without duplicative interfaces or repositories; migration of data into electronic medical record systems or related systems when needed; and ongoing maintenance, monitoring, and support of data interfaces after implementation.

The RFI further highlights interest in analytics, quality measurement, payer functions, care coordination, referral management, community connectivity, patient engagement, outreach, and virtual care. Questions must be submitted no later than July 29, 2026.
