The selected vendor will provide non-public Chapter 193 evaluation and determination services for a government authority in Lakewood, New Jersey. When a student is referred for an initial evaluation to determine eligibility for special education programs and services, a meeting involving the full child study team, the parent, and the regular education teacher familiar with the student’s performance must occur within twenty calendar days, excluding non-public school holidays, upon receipt of the 407-1 form to decide if an evaluation is necessary.
The service provider will conduct initial evaluations, which include classroom observation, teacher interviews, and at least two assessments conducted by at least two members in their qualified areas, with appropriate training or licensure. Other specialists may participate as required or determined necessary. The two assessments must focus on the areas of suspected disability, and while social assessments and outside assessments may occur, they are not counted toward the required minimum assessments.
