Must provide a notarized copy of the most current license/certificate/accreditation
Must provide a technical proposal containing all technical factors and sub-factors
Must provide a price proposal and completed Section B
Must submit a completed IRS Form W-14
Must provide proof of SAM registration including the SAM UEI number
