(1) Vendor needs to provide disease prevention and health promotion services.– Evidence–Based Health Promotion, Disease Prevention and Recreational Programs, which can provide education along with activities that support healthy lifestyles, promote healthy behaviors and improve functional status for older adults in our community in the hopes of preventing and reducing chronic disease and falls. – Targeted outreach with attention to; low income, low–income minority individuals including Native American, and older adults residing in rural areas, older adults with greatest economic need, older adults with limited English proficiency, older individuals who are frail or with severe disabilities older adults with Alzheimer’s Disease, or related disorders with neurological and organic brain dysfunction and the caretakers of such individuals to ensure that the most critical needs of the targeted population are met. – Evidence–based programs are defined as disease prevention and health promotion programs that are based on scientific evidence and demonstrate improved health and wellbeing or reducing disease, disability and/or injury among older adults. – Evidence–based health promotion programs may be delivered in a broad range of community locations. (2) All the questions must be submitted no later than December 02, 2025.
