(1) Vendor needs to provide case management system home and community–based services waiver services. Core system implementation shall establish the foundation 'core functionality' for the system, including intake, eligibility determination, person centered–service plan (PCSP) management, basic agency staff access, and the rollout of the public–facing website. Service delivery and enhancements shall expand system capabilities to support service delivery management, billing and claims processing, system enhancements to connexion, and improved functionality for agency staff and enhancements to the public–facing website. System enhancements shall focus on further enhancements to connexion, agency staff functionality, the public–facing website, and person portal. (2) The contract period will be for seven years. (3) Pre–proposal teleconference will be held on December 16, 2025.
