Seeking qualified firms to provide on-call bus safety video production services in California for a one-year term.

Provide on-call bus safety videography services
Possess requisite personnel and experience for services
Provide personnel listed as key personnel
Obtain prior written consent for key personnel changes or replacements
Submit resume and qualifications for replacement key personnel within 7 calendar days prior to departure
Invoice monthly on a Time and Expense basis
Document work completed in a monthly progress report
Maintain insurance coverage throughout the agreement term
Provide Commercial General Liability insurance with minimum limits of $1,000,000 per occurrence, $2,000,000 general aggregate, and $2,000,000 Products/Completed Operations aggregate
Provide Automobile Liability Insurance with a combined single limit of $1,000,000 per accident
Provide Workers' Compensation with limits required by the State of California, including a Waiver of Subrogation in favor of the Authority
Provide Employers' Liability with minimum limits of $1,000,000 per accident, $1,000,000 policy limit-disease, and $1,000,000 policy limit employee-disease
Provide Professional Liability with minimum limits of $1,000,000 if licensed or specially certified and Authority relies on performance
