This support is for the Medication Reconciliation Tool, fully integrated with the Veterans Health Information Systems and Technology Architecture (VistA) and Computerized Patient Record System (CPRS), which assists healthcare providers in efficiently reconciling patient medications across the VA’s continuum of care.

Services required include 24/7 help desk support by phone and email, access to IT product specialists, remote diagnostics, remote and on-site software fixes, quarterly version releases, patches, safety updates, and annual performance-enhancing updates. The MedRec Tool combines medications from multiple sources into a unified, intuitive display for comprehensive review and reconciliation, supporting both inpatient and outpatient environments. Integration with VistA/CPRS and support for VA-specific workflows are essential, as is the tool’s ability to generate patient-friendly medication lists for improved patient safety and clinical decision-making.

The selected contractor must provide continuous software support, including rapid issue resolution, regular software enhancements, and robust technical assistance as outlined. The contract encompasses both the software and related hardware (monitors and modules) supplied to the VA Healthcare System at the University Drive location in Pittsburgh, Pennsylvania.
