DESCRIPTION

A government authority in Hot Sulphur Springs, Colorado is seeking consulting and facilitation services to support a Community Access to Care Coalition. The selected vendor will help synthesize 18 months of community data and guide the coalition in turning high-level discussions into a practical, time-bound action framework.

The scope includes discovery and synthesis work, facilitated goal formulation, and the design and delivery of two high-impact action summits using modified results-based accountability workshops. The consultant will also facilitate the finalization and signing of memorandums of understanding or attainable action compacts among providers to establish clear accountability for specific access-to-care improvements.

In addition, the engagement includes leading a prioritization matrix session to rank community needs based on ease of implementation versus community impact. The overall objective is to help the coalition align stakeholders, prioritize needs, and advance actionable strategies to improve community access to care.
